The requester agrees to meet with members of the Missions Committee and/or submit a report documenting the use of the funds should the Committee request it.
The Missions Committee processes Applications for Mission Activity Funding at its monthly meeting. Completed forms must be received by the 5th day of the month.
Please note that this form is only a request for funding and its submission does not guarantee funding approval.
